Transitions™, a free community support program sponsored by Hospice of the Calumet Area, is dedicated to improving the quality of life through compassionate care for individuals and families facing a life-limiting illness. Decisions are being made and life is changing.
Transitions joins clients and caregivers at a time often filled with confusion and uncertainty.
Our support team brings relief by providing companionship, assistance with tasks and referrals to community resources. Funded with the help of community donations, Transitions services are provided at no cost.
